Brief Description of Instrument – Facilitated collection of data to describe the practice model employed in various settings and the factors that contributed to making these models work effectively. All site visits included interviews with the NP, physician and other members of the health care team, as available, such as managers, RNs, social workers and health educators. Site visits included Community Health Centres, long-term care facilities, Aboriginal Health Access Centres and Health Centres, Primary Care Networks, Health Service Organizations, emergency departments, hospital outpatients and other clinics, fee-for-service physician practices, public health units, Victorian Order of Nurses, Community Care Access Centres and other community agencies.

Scale Format – Site Visit, Key informant interviews, document review.

Administration Technique – Interviewer administered.

Scoring and Interpretation – Qualitative analysis.
